This Privacy Policy explains how we collect, use, share and protect information through this website (pkservice.org), our online inquiry forms, and our SMS text messages.
Once your child is in our care, their health information is protected by the Health Insurance Portability and Accountability Act (HIPAA) and is described in our Notice of Privacy Practices. If this policy and that notice differ about health information, the Notice of Privacy Practices applies.

It does not receive the answers you type into our inquiry form.

- What we send: appointment confirmations and reminders, scheduling updates, follow-up on your inquiry, and other service-related notifications.
- Data collected: your name, mobile phone number, and the consent you give when you opt in.
- Frequency and cost: message frequency varies. Message and data rates may apply.

You will receive one message confirming you have opted out, and we will remove your number from our list.

7. Children’s Privacy
This website is meant for parents, guardians, schools and other adults. It is not directed to children under 13, and we do not knowingly collect personal information online directly from a child under 13. Information about a child that a parent or guardian gives us is handled as this policy and our Notice of Privacy Practices describe.

We use administrative, technical and physical safeguards to protect information, including secure electronic health record systems, access controls, and HIPAA training for our staff. No method of transmission over the internet is completely secure. Text messages and ordinary email are not encrypted, so please do not send sensitive health information by text or email.

We keep inquiry information for as long as we need it to respond to you and provide services, and for as long as the law requires us to keep health and billing records. We keep your mobile number for text messages while you are subscribed. You may ask us to delete inquiry or text-message information at any time; we will do so unless the law requires us to keep it.
